Social Support Act (Wmo).
What is the Wmo?
The main purpose of the Social Support Act (Wmo) is to enable people to continue living at home and participating in daily life for as long as possible. Think for example of elderly people, people with disabilities or people with mental health problems.
The municipality of Son en Breugel ensures that these residents can receive support so that they can continue to live at home and remain active in society for as long as possible.
What can you do yourself?
The Wmo assumes your own strength and self-direction. This means that you first look at what you can do yourself to improve your situation. You may be able to ask family, neighbors or friends for help. You can also think about arranging help through your health insurance.
The municipality expects you to first try to find a solution yourself. Only if that fails can you apply for support. In Son en Breugel this is done via the cmd, where you can ask questions about Health, welfare, housing and work.
What does the cmd. do?
If you file a report with the cmd., they will look at your situation with you. In a conversation, you will discuss what you have already tried on your own and what you still need. The municipality can then help with, for example:
Help with the household
- Day care
- Adaptations in your home
- Guidance for independent living
How does it work?
- You report to the cmd.
You tell what you need help with. For example, with housework, transportation or daytime activities. - The cmd. examines your situation with you
Together we discuss what is going on and what you need to live and participate well. - It looks at what you can do yourself
The cmd. looks at whether you can arrange things yourself, or whether people around you can help. Only if that is not enough will professional help be used. - The cmd. looks to see if there is already existing help
Sometimes a (general) facility is already available, such as a grocery or meal service or Archipel's indication-free day care. - The cmd. looks at what help is needed
It determines what kind of help is needed, how often and for how long. We call this a customized Wmo provision. - A decision is made
You will receive an official decision stating what help you will receive and why.
These steps ensure that care is taken and that you receive appropriate help.
Measure Wmo provision: choosing between Health in kind or a pgb
If you need a customized service, you can choose how that help is arranged. There are two options:
- Health in kind (sentence)
The municipality arranges the help for you. You do not have to organize anything yourself. The help comes from an organization the municipality works with. - Personal budget (pgb)
You receive money from the municipality to buy help yourself. You choose your own care provider and make your own arrangements. You must be able to explain why you want this help and you must do the administration yourself.
What should you be able to do with a pgb?
If you want a pgb, you must be pgb-capable. Among other things, this means that you must:
- can arrange for help yourself and make good choices,
- can make clear arrangements with the caregiver,
- Be able to keep good records (such as bills and contracts).
The cmd. staff member will review with you whether you meet the Requirements .
Pgb not in own bank account
You don't get the money in your own account. The Social Insurance Bank (SVB) manages the money for you. You send the bills to the SVB, and they pay the caregiver.
The help must be good
The help you arrange with a pgb must meet quality requirements and at least be safe and of good quality. The employee of the cmd. checks this. So you cannot choose just any care provider.
Together we provide a solution
The Wmo is there to support you, but you remain responsible for your own life. The cmd. helps you if necessary, but also expects you to actively think along and participate. In this way we ensure that everyone in Son en Breugel can participate in society as well as possible.
