Reimbursement of health insurance deductibles and WMO copayments (for individuals with chronic illnesses or disabilities)

Residents with a chronic illness or disability may be eligible for financial assistance. This assistance covers a portion of the costs—up to €300 per year—for the health insurance deductible or the personal contribution under the Social Support Act (WMO).
You must apply for this assistance each year. 

Applications can be submitted at any time of the year. You must apply for the allowance after the end of the calendar year. For example, you must apply for the allowance for the year 2025 between January 1 and December 31, 2026. 

Apply in 3 ways: 

  1. Digitally via the button at the top of this page. In this case, you will also provide your supporting documents digitally. For this you can make a scan or a photo of the document.
  2. By phone at 0499 - 491491 (between 9 a.m. and 11 a.m.),
    after which you submit it by mailPostbus 8, 5690 AA Son en Breugel).
  3. Pick up a paper form and submit it to:
    - reception desk at Son en Breugel Town Hall (Raadhuisplein )
    - CMD Son en Breugel (Heistraat 6).

Requirements

You can get this allowance if you:

  1. On the date of your application you have been living in the municipality of Son en Breugel for at least 1 month (in accordance with registration in the BRP; the Basisregistratie Personen); 
     
  2. have paid your full health insurance deductible for the past 3 years (€385 per year), OR;
     
  3. currently pays a copayment to the CAK for a Wmo service; 
     
  4. received an income in the previous year that does not exceed the amounts listed below. Please refer to the amounts listed on your annual statement, provided by your employer or benefits agency.  

Written evidence 

The following supporting documents must be attached:

  • copy of bank card or copy of bank statement (your name and IBAN must be clearly visible);
  • the previous 3 years (example: if you apply for the reimbursement in 2026, submit the statements showing the deductible paid for the years 2023, 2024, and 2025),
    unless you already received the reimbursement last year, in which case only the statement showing the deductible paid last year is required;
  • the latest invoice from the CAK for the copayment for a Wmo service;
  • proof of your income from last year (annual statement(s) from your employer and/or benefits agency) and/or your partner’s income.
