Driving license lost or stolen

You can report the loss or theft of your driver's license in two ways:

• Report online free of charge to the RDW. You will need a DigiD for this. 
• By appointment at the counter. You will then fill out a missing person report and pay a €24.50 missing person fee.

Application after stolen/lost driver's license 

A replacement driver's license cannot be applied for online. 

• Applying for a replacement driver's license: by appointment at the counter.

• Block your driver's license (prior to your appointment)
  Block your lost driver's license online at the RDW(free of charge) or fill out a loss report during your appointment (€24.50).
   
• Bring to the appointment
  - color passport photo: no older than six months + meets passport photo requirements
  - valid ID
  - cash/debit card: you pay when you apply
  - under 18 years of age: a supervisor pass (to be requested via 2todrive) 
   
• Driver's license pickup (without appointment)
  You do not need to make an appointment to pick up your driver's license. 
  Do this within 3 months or you will have to reapply. 
  
  Excluded special/holidays can do so during our regular business hours:
  • Monday: 8:30 a.m. to 7 p.m.
  • Tuesday: 08.30-17.00
  • Wednesday: 8:30 a.m.-5 p.m. 
  • Thursday: 8:30 a.m. to 7 p.m. 
  • Friday: 8:30am-12:30pm

Rush request 

Driver's licenses applied for urgently before 2:30 p.m. can usually be picked up the next business day (officially this allows 48 hours). 

This is not possible online but only at the counter. Please make an appointment for this by phone at 0499-491491. 

For an urgent application, you pay an additional €39.65.

Health Statement

Have you lost a driver's license that required a Health Certificate when you applied? If so, keep in mind that your replacement license will have the same validity date as your lost license. Therefore, your replacement license will expire on the same date as the license you lost. 

Longer validity period

Would you like your replacement driver's license to be valid longer than the one you lost? Then you can apply for a Health Declaration. CBR will then reassess whether and for what period of time you may participate in traffic.
