GGzE Support
GGzE employs a mental health facilitator at cmd. A mental health facilitator is there for people who are mentally vulnerable and actively supports them.
What does a trailblazer do?
An outreach worker reaches out to people who may need help. This could include an existing GGzE client who needs assistance, but it could also involve, for example, a housing authority, a counselor, a social worker from the CMD, or even the police, who may indicate that someone does not seem to be doing well.
A case manager works with the individual to determine what is needed—such as counseling or treatment—and helps them apply to the appropriate facility.

Because a case manager at cmd. is on staff, they can quickly get in touch with other social workers and counselors to provide you with the best possible support.
