What is it?
Change form
You received a modification form at the start of your benefits. Are there changes in your family situation or, for example, in your housing situation? If so, you must notify the municipality.
For example, you can think about changes in the number of residents at your address, income, residence abroad, inheritance, car purchase and cohabitation/marriage.
Income Form
Do you receive benefits? And do you also receive other income? If so, you must inform the municipality via the button above ('pass on changes').
Consider, for example, income from (self-employment) employment, benefits, alimony, pensions, student loans and income from renting or subletting.
What to do.
Change form
- Please notify us of changes within 5 business days of the change.
- Failure to report changes, or to do so in a timely manner, will affect your benefits.
- If you are unsure about reporting the change contact the municipality.
Income Form
- Report your income to us after the end of the month.
- If you do not submit your income or do not submit it on time, entitlement to assistance cannot be determined. Your benefit will not be paid until you receive the form.
